For small Medicare home health agencies: send the OASIS assessment and the referral documents, and get the assessment checked against the chart, ICD-10 codes suggested and questions for the clinician — within one business day.

Agency with 60 active patients. A start-of-care OASIS and referral packet uploaded at 16:00. Next morning: codes suggested with the supporting text from the chart, two answers flagged as inconsistent with the nurse's notes, one question for the clinician. After her answer, the file is marked ready.
Choose your planIn a small home health agency the same few people do clinical work, scheduling and paperwork. Every start of care, recertification and transfer comes with an OASIS assessment and a set of diagnoses that need ICD-10 codes, and the assessment has to agree with what the clinician wrote in the chart. Many agencies send this review to outside reviewers and pay per file. OASIS Check does the same review at a lower price per file.
You upload the OASIS assessment together with the referral, the history and physical, and the clinician's visit notes. Files are transferred and stored in an encrypted account, and only the people you invite can see them.
Each answer of the assessment is compared with the documentation. Where an answer does not match what the notes describe, the item is flagged with the passage from the chart that raises the question. The review looks at the items that clinicians most often have questions about, such as functional status and the patient's history.
Diagnosis codes are suggested from the referral and the chart: a primary diagnosis and the secondary diagnoses that the documentation supports, each with the text it is based on. Your clinician or coder makes the final choice.
Questions for the clinician are collected in one list per file. The clinician answers them in the same place, and the file moves to ready once all questions are closed. The history of each question stays with the file.
The usual turnaround is one business day. Your dashboard shows every file and its status, so the office knows which assessments can be submitted and which still wait for an answer.
Pricing is per file, with no monthly minimum, so a small agency pays only for the assessments it sends. Larger volumes can move to a monthly plan.
To start, you invite the people who will upload files and answer questions, and send the first file the same day. No software needs to be installed; the dashboard works in the browser.
Reviewers follow the same checklist for every file, so the quality of the review does not depend on who happens to be available that day. Over time you also see patterns across your files: which assessment items are flagged most often and which clinicians receive the most questions. That helps you plan training for your team where it has the most effect, instead of correcting the same issues file by file.
OASIS Check is a review service. It does not submit assessments or claims, and final clinical and coding decisions remain with the agency and its clinicians.
